We're looking for an organised, proactive and people-focused Care Coordinator to join our Somerset & Wiltshire team in Frome.

What will you be doing?
As Care Coordinator, you'll play a key role in the day-to-day delivery of our service across Somerset & Wiltshire.
You'll bring together people, schedules and information to help ensure our clients receive high-quality, dependable care.
Your responsibilities will include:
- Building, managing and optimising Care Professional rotas.
- Matching Care Professionals with clients to promote continuity and positive relationships.
- Coordinating new care packages and supporting successful client starts.
- Managing Care Professional availability, working patterns and scheduling requirements.
- Responding quickly and calmly to sickness, absence and last-minute changes.
- Arranging emergency cover while minimising disruption for clients.
- Working closely with Care Managers, Team Leaders and other branch colleagues.
- Coordinating training, shadowing and other operational requirements.
- Developing positive relationships with clients, families and Care Professionals.
- Helping identify potential gaps in capacity before they affect service delivery.
- Participating in the branch on-call rota.
- Supporting the continued growth and success of the Somerset & Wiltshire service.
This isn't simply about filling gaps on a rota.

You'll get to know our clients, understand the strengths and availability of our Care Professionals and use that knowledge to help create care arrangements that work for everyone.
What are we looking for?
You don't need to tick every box, but we'd particularly like to hear from you if you have experience within domiciliary care, homecare, healthcare scheduling, care coordination or another fast-paced people-focused environment.
You'll ideally bring:
- Excellent organisation and planning skills.
- Strong communication and relationship-building abilities.
- Confidence managing competing priorities.
- A practical, solutions-focused approach.
- The ability to remain calm when plans change.
- Good IT skills and confidence learning scheduling and care systems.
- Strong attention to detail.
- A genuine commitment to providing an excellent experience for clients and colleagues.
- Previous experience within health or social care would be advantageous.
- A Level 2 or Level 3 qualification in Health & Social Care would be desirable.
